Proof of Residence (one of the following)

• Persons with proof of residence in Clay or Platte counties.
• Documentation from a shelter where the client is a current resident.
• Letter postmarked within 30 days and addressed to client at the home of a friend or relative.

Personal identification and proof of need
First visit or 12 month re-verification documentation includes:

• Driver’s license or state issued identification card AND Social Security card for all members of the household                               
• Proof of address (current utility bill or rent agreement) dated within past 30 days;                                
• Proof of income (letter; check, disability or unemployment stub, bank statement with deposit noted).
• Utility bill, eviction notice or other documentation indicating the financial aid assistance to be requested by the client.
